How To Use The CIBA Application

You will need Adobe Acrobat Reader version 4.0 or greater. If you do not
have this program you can download a free version of the program from
their web site at: www.abobe.com

Once you have opened the "CIBA Application" in Adobe Acrobat Reader
you may tab through the fields and fill in the form or you may print out the
form to complete by hand. Once completed, please email the application to
CIBAQuote@cibaservices.com.

TIP: To save your completed application, select "Save As" from the File
menu. Remember to give your file a different name when you save it.
